Refreshment Sponsorship Form

 Refreshments, in the form of cookies, for social hour can be sponsored by individuals or groups, any Sunday except Soup Sundays. To sponsor refreshments, please fill out and submit this form. Then either pay in the FUS office or sumbit payment using the Donate Now button on the web page, entering in refreshments in the comment field. 

• Sponsorship is $35 for each Sunday.
• Sponsorship is on a first-come first-served basis. Sign up early to reserve your specific date.
• Dates are reserved when payment is received.
